To expedite this process, completely fill the application out going ten years back on employment history, if you have been driving a commercial vehicle that long or longer. We will need physical address(es) and telephone & fax numbers going back at least 3 years for past employment. Request for Check of Driving Record, just fill out the top half of the form. Request for Information for Previous Employer, just what is highlighted in yellow, please. Once completed with the forms fax them to us at (608) 938-3364 and mail the original to Preferred Transit, Inc. - 300 S Pratt Rd - Monticello, WI 53570.
